Immunizations

Immunizations are required by Arizona State Law (A.R.S. 15-872) and documentation provided prior to beginning school.

Required Immunizations are:

Preschool (up to age 59 months):

• Diphtheria, Tetanus, and Pertussis
• Polio
• Varicella
• Measles/mumps/rubella (MMR)
• Hepatitis B
• Hepatitis A
• HIB (Haemophilus influenzae type b)

Kindergarten- 12th grade (age 4 and up):

• Diphtheria, Tetanus, and Pertussis
• Polio
• Varicella
• Measles/mumps/rubella (MMR)
• Hepatitis B
• Prior to beginning 6th grade (age 10 and up): Meningococcal and Tdap (Tetanus, Diphtheria, and Pertussis booster)

Any of the listed above immunizations a student receives throughout the year, need to be reported to the school nurse by providing documentation; NO immunizations are administered by the school nurse at any time in HUSD.
