Welcome to Humboldt Unified School District #22!
We're excited to have you join our team! To start the New Hire process, the first step is to have an electronic action request from the site where you interviewed. Once we receive this, we will send you the necessary paperwork via email to be completed and returned. Afterward, we will schedule a meeting with Human Resources for your onboarding.
Below are the required documents needed to complete your hiring process. These will be sent to you by the HR department and must be filled out and returned before your first onboarding meeting. Please ensure that your name matches the name on your Social Security card and, if applicable, your teaching certificate. Required Documents for All Staff:
- Government-Issued Photo ID (Driver's License or Passport)
- Social Security Card
- Immunization Record (Mumps, Measles, Rubella shot record)
- VOID Check or Bank Verification Form (for Direct Deposit setup)
- I-9 Verification Form (included in packet)
- Direct Deposit Form (included in packet)
- State and Federal Tax Withholding Forms (W-4 & A-4) (included in packet)
Additional Documents for Certified & Professional Staff:
- Official Transcripts (sent directly to the District Office)
- Validated Certificate (must be recorded at the Yavapai County ESA office prior to meeting with HR)
- Prior Experience Verification Form (if applicable)
Required Fingerprint Clearance for the Following Staff:
- Certified Staff
- Professional Staff
- Administrative Staff
- Classified Title 1 Aides
- Classified Bus Drivers
- Classified Preschool Aides
