School Meal Applications

It's More Than Just an Application!The free/reduced meal program impacts more than just school lunches. It can help with school funding, college application fees, SAT fees, sports fees, and more. Even if your child doesn't plan to eat school meals, completing the application ensures that our schools and families get the resources they need.

All HUSD Families Are Asked to Complete the Application or Opt-Out

  • You can get application forms at your school office, the Child Nutrition Office at 6901 Panther Path, Prescott Valley, AZ, or any school cafeteria.

Frequently Asked Questions

Do I need to fill out an application for each child?

  • No, one form covers multiple children in the same household.
     

Who will know our family submitted an application?

  • Our system is confidential. Even if your child qualifies, they will follow the same process as any other student, whether they eat lunch or not.
     

Do I still need to fill out a form if I qualify for other state benefits?

  • If you received a letter from HUSD Child Nutrition stating your child qualifies, you do not need to apply.
     
  • If you receive state benefits but haven’t received a letter from us, you will need to apply for free meals.
     

Do I have to provide a social security number?

  • If you don’t have one, check the box saying you don’t have one. If you do, only the last 4 digits are needed, and this information is kept confidential.
     

Important Information

Parents must apply every school year. Even if your child won’t be eating school meals, applying helps ensure our school district receives all available resources.

To apply, an adult household member must complete the application, which should include all children in the household. Eligibility depends on household size and income, with children in households at or below the USDA set income levels qualifying for free meals. Foster children and those receiving SNAP, TANF, or Food Distribution benefits are eligible for free meals.

Note: It’s your responsibility to ensure the application is submitted to the Food & Nutrition office. Processing can take up to 10 days, and benefits begin only after approval.

Print the application, fill it out completely, sign it, and return it to the Child Nutrition Office at 6901 Panther Path, Room 128, Prescott Valley, AZ 86314 or your child’s school. If you have questions, contact Denise Vaughn at 928-759-5013.

Applications are available in English and Spanish at your child's school and the cafeteria.
