Step 1 
The GHMS office will have the enrollment packet to register your student.

Step 2 
Our office staff will assist you and answer any questions you may have.

Step 3 
You may return the completed enrollment packet along with a birth certificate, updated immunization record, withdrawal form and subjects/grades from former school to GHMS.

Step 4 
Paperwork will be processed that day and student will start school the following day.

Immunizations:

• Arizona State Law requires that a documented immunization history be presented at the time of enrollment. All immunizations must be current.
• A valid record can be one given to parents by their doctor or clinic. For children transferring, a copy of the school record is sufficient.
• The student must be immunized against diphtheria, pertussis (whooping cough), tetanus, polio, measles, mumps, rubella, varicella (chickenpox), and  hepatitis B.

If there is a medical reason that this treatment is inadvisable or if immunizations are against the personal beliefs of parents, an exemption must be completed and signed. School personnel will refer parents without documented records to their doctor or health department before completing the registration process.
