Parent / guardian, please contact the front office at (928) 759-4300 for registration information.

Along with registration paperwork to be completed, the following documentation will need to be provided by the parent to expedite a speedy enrollment process:

• Official withdrawal form from previous school attended
• Current immunization record copies which, by law, MUST be provided prior to entering the classroom
• Copy of certified birth certificate
• Proof of residency within boundaries. Proof must contain enrolling parent / guardian’s name and address (ie: utility bill, rental agreement, etc.)
• Notarized guardianship/temporary custody document if applicable
• Court documents…if there is sole/restricted parental contact by court order

Please advice us at the time of registration if your child needs any of the following services:

• Special education support services, documents required
• Medical support services, documents required
• Language support services (see form included in enrollment packet).
