IMMUNIZATION REQUIREMENTS

KINDERGARTEN IMMUNIZATIONS

Immunizations are required by Arizona State Law (A.R.S. 15-872). Required immunizations for kindergarten are: Diphtheria, Tetanus, Pertussis, Measles/ Mumps/ Rubella (MMR), Polio, Varicella (chicken pox), and Hepatitis B. Please schedule an appointment with your child’s doctor to review their immunizations. You may also schedule an appointment with the Yavapai County Health Clinic at 928-583-1000. Kindergarteners are required to have their immunizations before the first day of school. If you have any questions regarding immunizations please call the Granville Elementary Health Office at 928-759-4810.

CHILDREN 11 YEARS AND IN 6^TH GRADE IMMUNIZATION REQUIREMENTS

In accordance with state law, all children at the age of 11 and going into 6^th grade must have the following before they can be assigned a 6^th grade class. Diphtheria, Tetanus, Pertussis, Varicella (chicken pox), and meningitis. Please visit the HUSD website (humboldtunified.com) for specific requirements. Any new immunizations your child receives throughout the year should be reported to the school nurse. During the year when your 6^th grader turns 11 please schedule their appointment with their doctor and bring their immunization record to the health office. You may also schedule an appointment with the Yavapai County health Clinic at 928-583-1000. If you have questions regarding immunizations please contact your child’s doctor or you my call the Granville Elementary Health Office at 928-759-4810.

Partners For Healthy Students

The Partners for Healthy Students Program is a free healthcare service available to uninsured and underinsured (with a high deductible) school students in our school district. This program is provided through Yavapai Regional Medical Center. Clinics are held the 2nd and 4th Tuesday at Granville starting in September. Services include: Diagnosis and treatment of illness, Prevention/well child visits, Sports physicals, Prescription Medicine, when required, Laboratory tests and radiology services, Referrals to doctors, dentists and other specialists, Health education for safety, nutrition and dental hygiene.

Healthcare visits are by appointment only and parent involvement is required. You may contact your school nurse at your child's school for more information, or you may call 928-771-5662 to schedule an appointment.

GES TRAFFIC PATTERN - Morning Drop off in back of school!

Starting Monday morning 9/28/09 everyone will notice a change in our traffic pattens for buses and for student drop-off and pick-up.
